Overview
Deaths must be reported to the coroner in the following scenarios:
- The cause of death is unknown
- The patient was not seen or treated for an illness 28 days before they died
- The death was sudden or unexpected
- The death was within 24 hours of admission
- All deaths of people under 18 years of age
- Deaths that may be linked to treatment, surgery, or anaesthetic
- Deaths that may be linked to accidents, however long ago they happened
- Deaths that may be linked to drugs or medications, both prescribed or illicit
- If suicide was a possible cause
- If there is suspicion regarding the history of death or there is a history of violence
- Accidents or injuries
- Deaths relating to occupation e.g. asbestos exposure
- All deaths of people in custody or detained under the Mental Health Act, even if they died of natural causes
- Deaths due to ill-treatment or neglect
- Deaths due to starvation
- Stillbirths if there is doubt as to if the child was born alive or not
- Prisoners
- Disability pensioners
