Overview
A benign cause of dilated pupils most commonly seen in women. It is usually due to damage to the ciliary ganglion or postganglionic parasympathetic nerve fibres, which are usually due to viral or bacterial infection, such as Herpes zoster ophthalmicus.
Features
- Pupil may be dilated in the early stages and may be irregular
- Pupil reacts slowly to light but briskly to accommodation
- Once the pupil constricts, it stays constricted for an abnormally long time
- Most cases are unilateral
- Holmes-Aide syndrome is where there is associated absent knee/ankle reflexes
